TO: ALL EMPLOYEES

ELIGIBILITY: New employees are eligible to take vacation time at six
months after the end of their probationary period (or at 9 months of
continuous employment).
Vacation time is scheduled from March to March each year. If you plan to
take vacation time between March 2005 - March 2006, please complete the
back of this form and return it to Anne Marie at the office by March 31,
2005.
The Personnel Coordinator will review each request and send written
confirmation (the original of your request form) to employees not later
than April 30, 2005. Confirmation will be included in pay-stub envelopes
on that payday.
Any requests for vacation or short-term leave submitted after March 31,
2005 will be subject to Managers’ approval and availability of
appropriate relief coverage. These requests are subject to the time-line
outlined under Short-Term Leave in the Personnel Policy & Procedures
Manual. Manager’s approval is required prior to coverage of any shifts
outlined in the request for time off.

vacations, please refer to the Personnel Policy & Procedures manual.
